Ask to see the birth certificate and check if it is issued by National Population Commission, then circle the appropriate corresponding code, noting whether or not the certificate was seen. If the child has a birth certificate issued by National Population Commission and it was seen, circle '1' and skip to BR5. If the child does not have a birth certificate or the birth certificate seen is not issued by National Population Commission then, circle '3'. If the child has a birth certificate but the mother/primary caretaker is unable to show you the certificate ('2'), continue to the next question. If don't know circle 8.
